Church of God (Holiness) minister\’s and Christian workers who wish to have their names and personal contact information printed in the 2015 Minister\’s Roster, must fill out and send in the Credential Certificate form . The due date is July 1, 2015. A $15 fee must accompany the form. The form should be sent to Mar D Avery, Secretary, PO Box 4060, Overland Park, KS 66204.
